How to Register Your Child

REQUIRED DOCUMENTS FOR REGISTRATION

For Kindergarten students and students who are new to the district stop by the school office to pick up a registration packet to register your child between the hours of 8:00 am and 2:30 pm, Monday through Friday. The following items are needed for registration:

  • Official Birth Certificate
  • Proof of Residency to include one of the following: Military Housing Order, a recent utility bill (Telephone, SDGE or Water), or a recent cable bill
  • Up-To-Date Immunization Record

For students coming from within San Diego City Schools, the office needs a copy of your proof of residency only.

Immunization Requirements for School Entry:

POLIO- 4 doses (3 doses are enough if at least one given after the 4th birthday)
DTP/DTa- 4 or more doses (1 more dose is needed if the last dose given before the 4th birthday)
MMR- 2 doses for Kindergarten entry
HEP-B - 3 doses for Kindergarten entry
VARICELLA - 1 dose or proof of disease on a shot record (required for Kindergarten students and students coming from outside California)

Eligibility: If you have a child who will be turning 5 years of age on or before September 2, 2018, and you live within the Miller Elementary School Boundaries, you may wish to enroll your child in Kindergarten for the 2018-19 school year. Children who turn 5 after September 2 and on or before December 2, 2018 may choose to enroll in Transitional Kindergarten at Miller for the 2018-19 school year.

Transfers: Please provide the previous school address and your signature for release of student's records.

Emergencies: When completing the enrollment form, it is very important that the name, address and telephone number of the person(s) to call in case of an emergency is included.
