Thank you for inquiring about enrollment at Miller Elementary. Due to the emergency school closures, the new student enrollment process for the 2020-21 school year will be completed electronically.

If you are unable to complete the enrollment online, you can register your child in-person at our school office on Tuesdays from 8am-2pm.

Required Enrollment Documents

Please complete the following forms and email them to LRAHER@sandi.net

  1. Birth Certificate/Passport
  2. Immunizations (up to date)
  3. Proof of Residency (recent utility bill, rental agreement or rent payment receipts, property tax payment receipt, mortgage documents, or military housing orders)
  4.  IEP (if applicable)
  5.  Online Registration Form (only for students new to SDUSD) OR Enrollment Form
  6.  Home Language Survey
  7.  Health Information Form
  8. SDIR (Immunization Registry)

Transitional Kindergarten

Only Students who turn five (5) years between Sep. 2 and Dec. 2, 2020 for the 2020-21 school year are eligible for TK. If your child turns five (5)



Only students who are five (5)  on or before September 1st, 2020 are eligible.


Required Immunizations for Elementary School

    • 5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday)
    • 4 Polio (3 doses meet requirement if at least one was given on or after the 4th birthday)
    • 3 Hep B
    •  2 MMR (Both doses given on or after 1st birthday. Only on dose of mumps and rubella vaccines are required if given separately.)               
    • 2 Varicella
